HEORTLAND dualified Professional Heartland PCA, a home health care company providing personal care assistance, is seeking a qualified professional (Registered Nurse, Licensed Social Worker, Mental Health Professional, or a Qualified Developmental Disabilities Specialist to serve it's Bemidji office Responsibilities Include: Develop care plans for clients by utilizing the county assessment. · Train and evaluate new PCAs Oversee the delivery of PCA services Ensure client health and safety needs are being met Maintain required documents noting changes in condition and requesting reassessment as needed Assist with PCA Choice supervision as requested by DHS regulations and the participant/responsible party.
